New Patient Forms
All new patients will need to complete this paperwork prior to their first appointment. The forms will be submitted electronically to the Office.
Health History Forms
All patients will need to update their health history forms annually.
Minor Consent Form for Routine Care
Please complete this form if you are unable to accompany your minor child to their routine care (cleaning, xrays, fluoride) visit. This form does not cover any other treatment and will remain on file until your minor child reaches the legal age of consent in the State of Ohio. If your child will be receiving more extensive treatment and you are unable to accompany them for their visit please speak to the front desk team to get the appropriate paperwork.
